Oracle carries out an examination to determine what caused the failure, said the VA. Oracle did not immediately respond to CNBC's request for comment.

The failure is the latest stumbling from Oracle in a thorny, years of honor rollout with the VA, which was affected by the security concerns of the patient. In 2021, the agency hired a strategic review of Cerner in 2021 and temporarily stated the provision of the software in 2023.

Four VA facilities in Michigan are to use Oracle's federal government in 2026.
